RHP: Add node to a cluster, scale RAC DB to the new node

Rapid Home Provisioning can extend a cluster to additional nodes, and scale a RAC Database to additional nodes. This demo shows both of those operations.

RHP can also remove DB and GI homes to scale a RAC cluster down, but we don't go through those (very similar) steps here.
